In order to obtain a duplicate of a document on higher education and/or a supplement to it, it is necessary to submit a written application to the institution of higher education that issued the document. This can be done in person or through an authorized representative.
If the institution where one studied has been reorganized, one must contact its legal successor. These are cases of joining, merging or liquidation of a higher education institution.
If there is no legal successor or the institution does not carry out educational activities (the license has been revoked, it has not been moved from the uncontrolled territory), then the application must be submitted to the Ministry of Education and Science of Ukraine. The Ministry will determine the institution that will issue the duplicate.
In the application, it is necessary to specify:
- full name, date of birth;
- series and number of the document certifying identity and citizenship;
- place of residence, phone number;
- the name of the higher education institution and the date of its completion;
- the name of the document, a duplicate of which is ordered;
- the reason for ordering a duplicate;
- other information one considers important;
- consent to the processing of personal data.
Applications to the Ministry of Education and Science of Ukraine should be submitted to the following address: 10 Peremohy Ave., Kyiv, 01135 or to the email ez@mon.gov.ua.
The information in the duplicate is reproduced on the basis of the data contained in the Unified State Electronic Database on Education (EDEBO). Information about documents began to be entered by higher education institutions into the database in 2000.
If there is no information about the fact of the issuance of a diploma in the EDEBO, then the higher education institution can use a copy of the document to confirm this, as well as an archival certificate of study and an extract from the journal of issuing documents about higher education or an act of destruction of the primary document about higher education.
If there is no information about the fact that the diploma was issued in the EDEBO, as well as the archives of the higher education institution are not available, then one can:
- apply to the court – it must establish the legal fact of obtaining the relevant degree or level of higher education;
- confirm the receipt of the diploma with an official letter from the relevant competent authority, in which the fact of affixing the “Apostille” stamp of the Ministry of Education and Science of Ukraine or consular legalization is confirmed.
Therefore, it is necessary to submit an application to the Ministry of Education and Science of Ukraine to determine the institution that will issue a duplicate. A copy of the court decision or an official letter confirming the apostille must be attached to the application.
If it is necessary to restore a diploma obtained in the non-Ukraine-controlled territory of Crimea or Donbas before 2000, the procedure is the same as described above. If we are talking about the production of a duplicate of a document issued from 2000 to 2014 and entered in the EDEBO, then:
- applicants from the Crimea should apply in writing to the Ministry of Education and Science to determine the institution that will issue a duplicate;
- applicants from the Donbas must submit a written application to the legal successor or the higher education institution displaced from the non-controlled territory. Otherwise, submit a written application to the Ministry of Education and Science to determine the institution that will issue a duplicate.
A copy of the primary document (diploma and its supplement) must be attached to the application.
